To use the online form, follow these steps:
- Access the Form: Scroll down below this guide, and you will find the form ready for you to fill out. Alternatively, access the form directly in your browser.
- Select a Service: Choose the specific service you require from the Lab Support Desk. You may need to select 'Show more services' to view the complete list of available services.
- Choose Appointment Date and Time: Select a suitable date and time for your appointment. Appointments can be made up to two weeks in advance.
- Add Your Details: Begin by providing your name, SMC email address, and any other relevant details.
- Additional Comments: Include any specific concerns or requirements you would like the support team to be aware of in the notes section. Answer any additional questions as prompted; this will help us prepare in advance.
- Submit the Form: Once you have filled in all the necessary information, review it for accuracy. When ready, click the "Book" button at the end of the form to send your appointment request to our team. You will receive a confirmation email containing information on how to attend your meeting. If you don't see your confirmation email, remember to check your Spam or Junk folder.
- Join the Zoom Meeting (if applicable): If you have schedule a meeting via Zoom, use the Language Lab Support Desk Zoom meeting link to join your virtual appointment.
